Heart & Vascular Center of West Tennessee and Rhythm are committed to providing you with continuous, expert care directly from your home, and we take our responsibility to you very seriously.
Automatic Monitors or Mobile App
If your device is equipped with an automatic monitor, please ensure it remains plugged in at all times. This allows us to receive data from your cardiac device routinely and maintain frequent data collection.
Manual Monitors
If your device uses a manual monitor, you will receive a reminder call and/or text message a few days prior to your scheduled appointent date.
Support and Assistance
Should there be any concerns regarding the data received from your home monitor, the providers at Delta Clinic-Heart & Vascular Center of West Tennessee will contact you directly. For any inquiries about using or maintaining your monitor, please contact our support team for help.
