Please arrive at least 15 minutes before your scheduled appointment time to allow sufficient time for check-in and any necessary paperwork.
For each visit, please bring:
Your insurance card
A valid photo ID
A current list of medications you are taking
If you are a new patient, you may complete your paperwork before your appointment. Download and print the new patient forms using the link below and bring the completed forms with you to your visit.
Parking, Accessibility & Campus Information
Our Lexington office is located within the Lexington Professional Building off Highway 412.
Our location offers a large parking lot with convenient handicap-accessible parking spaces near the front entrance. A designated patient drop-off area is also available for those who may need assistance accessing the building. Our offices are located downstairs, while the upstairs space is occupied by a podiatry clinic.
